Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Charlotte Center for Cosmetic Dentistry, operating as Destination Smile (“the practice”), is required by law to protect the privacy of your protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

Treatment

We use your health information to provide and coordinate your dental and aesthetic care, and may share it with other providers, specialists, or laboratories involved in your treatment.

Payment

We may use and disclose your information to bill and obtain payment for services, including verifying coverage and processing claims with your dental or insurance plan.

Health care operations

We may use your information to run the practice — for example, quality review, training, scheduling, and administrative functions.

Appointments & communications

We may contact you to provide appointment reminders or information about treatment options, using the contact details and preferences you provide.

Uses that require your written authorization

Most uses and disclosures not described above — including most marketing, the sale of PHI, and the use of any photographs or before-and-after images — will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.

Your rights

• Access. You may inspect and request a copy of your health information.
• Amendment. You may request that we correct information you believe is incomplete or inaccurate.
• Accounting of disclosures. You may request a list of certain disclosures we have made of your information.
• Restrictions. You may request limits on how we use or share your information, including a restriction on disclosures to a health plan for services you pay for out of pocket in full.
• Confidential communications. You may ask us to contact you in a specific way or at a specific location.
• Paper copy. You may request a paper copy of this notice at any time.
• Breach notification. You have the right to be notified following a breach of unsecured protected health information.

Our duties

We are required to maintain the privacy of your health information, provide this notice of our duties and privacy practices, and abide by the terms currently in effect. We reserve the right to change this notice and to make the revised notice effective for information we already have, as well as information we receive in the future. A current copy will always be available in our office and on this website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with the practice using the contact below, or with the U.S. Department of Health & Human Services. You will not be penalized or retaliated against for filing a complaint.
